St. Joseph's Medical Center and American Heart Association want to recognize people who have made positive changes to improve their quality of life and health. No change is too small, and every accomplishment is significant.

That’s why we are now accepting nominations for the 2015 Lifestyle Change Award, locally sponsored by St. Joseph's Medical Center. Nominations for the Lifestyle Change Award are open to the public and can be submitted by friends, co-workers, relatives…you can even nominate yourself!

Nominations will be accepted until Wednesday, September 18.
